Care Navigator

Responsibilities

Spring Mountain Treatment Center is a 110‑bed mental health facility providing acute crisis stabilization services for adults, teens, and children in a secure inpatient setting since 2001. Our individualized treatment includes a structured, therapeutic environment, intensive physician involvement and oversight, and daily group and educational programs. We provide a caring and healthy environment that gives individuals a chance to make positive changes.

We are currently seeking a dynamic professional to join our team as a Care Navigator. This role is responsible for meeting with all eligible patients to discuss the benefits of outpatient services as part of their continuum of care treatment. The Care Navigator communicates eligibility to key members of senior leadership and associated departments. They are responsible for routine collection and data entry of outpatient eligibility into Midas for trending report purposes.

They should have a thorough understanding of the Step‑Down Playbook, implementing components as needed and workflow process as it pertains to case management and discharge planning. Presentation and customer service skills are key.

• Provide quality case management and psychotherapeutic services to Spring Mountain population.
• Provide social histories, discharge planning, case management and psycho‑educational groups to age‑specific populations.
• Provide family meetings as needed.
• Consult with Social Services, Utilization Management and Outpatient on patient eligibility.
• Perform other duties as assigned or required by this position.

• Bachelor’s degree in mental health or related field preferred
• Minimum two (2) years’ experience working in psychiatric/mental health field. Possess knowledge of clinical issues related to substance abuse patients and their discharge planning
• Basic Life Support certification required
• Ability to follow both written and verbal instructions
• Understand and honor the established hospital rules, regulations and policy and procedures
• Good communication skills, i.e., reading, writing, and speaking
• Good leadership qualities which include self‑control, patience, and empathy
• New hires are required to attend one full week of New Employee Orientation on day shift, normally 8:00 am – 4:30 pm Monday through Friday
• Must successfully pass background check (includes criminal history, child abuse and neglect registry, and ABI/FBI fingerprint check)
