Work Schedule
:

Ability to work full-time (Mon-Fri, 8:30-5:00) must have schedule flexibility due to organizational events.

Attendance:

JOB SUMMARY:

Case managers at HFP are responsible for encouraging, guiding, supporting, and monitoring the success of program participants. Case managers may specialize within the organization working on initial intake and support services to assist clients in adjusting and stabilizing or work with clients proceeding to career and next step goals. Must have knowledge of barriers to entry into the workforce for the re-entry population and problem-solving approaches to assist clients with gaining employment.

Case managers deliver services with a client- first focus within a team setting. The Case Manager/Career Advisor has a pivotal role on the team that will provide career counseling and coaching to assist clients enhance their training and professional skillset to obtain employment in their desired area. This position will be responsible for managing and supporting a caseload of clients as they navigate their employment journey.

Experience with case management with the reentry population, coping with high-stress at-risk clientele, and conflict resolution.

Job Duties & Responsibilities

Coaching & Career Services

• Develop career plans with participants that map the steps necessary to reach short- and long-term career and/ or academic goals.
• Utilizes industry standards to advise clients toward careers of interest and growth.
• Conducts industry-focused assessments to gauge their readiness in various career pathways.
• Maintains continuous contact with customers in caseload to provide on-going career services, case management, career advising, and regularly monitors progress to ensure completion of academic or occupational goals.
