Company Description: Nevada Senior Services (NSS) is a long-standing, trusted provider of compassionate, person-centered care for older adults, people living with disabilities, and the caregivers who support them. Hospital 2 Home (H2H) is a program of NSS serving the vulnerable population with support services to remain living safely and comfortably at home.
Role Description: The Community Social Worker is a full-time role based in the Las Vegas Metropolitan Area. This role involves conducting psychosocial assessments, developing and implementing care plans, and coordinating services for older adults, people with chronic disease, disabilities, and their caregivers. Daily activities include providing case management, resource referrals, benefits navigation, and advocacy to help participants remain safely and comfortably in their homes or communities.
The Community Social Worker collaborates with interdisciplinary teams and community partners, leads or supports educational workshops, and documents services in accordance with agency and regulatory standards. The role also requires outreach to underserved communities, ongoing follow-up to monitor progress, and participation in program evaluation and quality improvement efforts.
Qualifications
- Strong case management and assessment abilities, including care planning and advocacy skills.
- Community outreach, resource coordination, and familiarity with aging, disability, and caregiver support services.
- Effective communication, interpersonal, and conflict resolution skills for working with diverse individuals and families.
- Organizational, time management, and documentation skills, including proficiency with electronic records and basic office software.
- Knowledge of social work ethics and culturally responsive practices.
- Must be a kind, confident, patient, and independent person with the ability to work in a hybrid environment and collaboratively within multidisciplinary teams.
- Bachelor’s degree in Social Work, Gerontology, Psychology, Human Services, or a related field; LSW or LMSW licensure/certification preferred.
- Experience working with older adults, people with disabilities, or caregiving families in community-based or healthcare settings is highly beneficial.
- Must have reliable transportation for community visits within the Las Vegas Metropolitan Area and the ability to meet any applicable background check requirements.
