Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents and supervises nursing assistants in accordance with federal, state, and local regulations
, as well as facility policies.

This role is essential in ensuring clinical excellence, resident safety, and positive outcomes
.

Qualifications
  • Graduate of an approved LPN program
  • Current, active, and unencumbered PA LPN license
  • Current CPR certification
  • Experience in long‑term care preferred
  • Knowledge of rehabilitative/restorative nursing practices preferred
  • Experience with Point Click Care  (PCC) or EHR systems preferred
  • Strong communication and teamwork skills
  • Must meet all background, health, and compliance requirements
Key Responsibilities
Clinical Care
  • Administer medications and treatments per physician orders
  • Maintain accurate documentation in MAR/TAR and Point Click Care
  • Monitor and report changes in resident condition
  • Perform clinical tasks including:
    • Vital signs and accuchecks
    • Specimen collection
    • Wound care and treatments
Resident Care & Coordination
  • Participate in admissions, transfers, and discharges
  • Communicate with physicians and families regarding resident status
  • Collaborate with the interdisciplinary team (IDT)
  • Assist with care planning and quality improvement initiatives
Supervision & Leadership
  • Direct and supervise CNAs and nursing assistants
  • Provide coaching, guidance, and support to staff
  • Ensure adherence to policies, procedures, and standards of care
Compliance & Safety
  • Ensure proper handling and documentation of controlled substances
  • Follow infection control, PPE, and safety protocols
  • Complete incident/accident reports per policy
  • Maintain a clean, safe, and compliant work environment
Resident Rights Commitment
  • Maintain strict HIPAA compliance and confidentiality
  • Treat all residents with dignity, respect, and compassion
  • Respect resident choices and report concerns appropriately
  • Ensure privacy and culturally competent care
Physical Requirements
  • Ability to lift, push, and pull 25–50 lbs
  • Ability to move throughout the facility during shift
  • Ability to respond to emergencies and assist with evacuations
