- Receive, review, and manage reportable incidents across assigned programs.
- Conduct thorough investigations in accordance with organizational policies, state regulations, and federal guidelines.
- Interview staff, individuals served, families, witnesses, and other stakeholders using trauma-informed practices.
- Prepare detailed, objective, and timely investigative reports and supporting documentation.
- Collaborate with program leadership, Human Resources, and other departments regarding investigation outcomes and corrective actions.
- Monitor incident trends, identify root causes, and recommend quality improvement initiatives.
- Participate in incident review committees and follow-up planning activities.
- Assist in developing and monitoring corrective action plans to ensure successful implementation.
- Serve as a subject matter expert on incident reporting systems, documentation standards, and regulatory requirements.
- Provide training and support to staff on incident management processes and best practices.
- Maintain professional relationships with regulatory agencies and external stakeholders.
- Travel to program locations as needed to conduct on-site investigations and reviews.

- High School Diploma or GED and extensive experience in incident management, investigations, behavioral health, or human services
- Valid driver's license with a clean driving record

- Strong written and verbal communication skills
Proficiency with Microsoft Office and electronic documentation systems
- Bachelor's degree in Human Services, Criminal Justice, Social Work, Psychology, or a related field
