What You’ll Do
  • Receive, review, and manage reportable incidents across assigned programs.
  • Conduct thorough investigations in accordance with organizational policies, state regulations, and federal guidelines.
  • Interview staff, individuals served, families, witnesses, and other stakeholders using trauma-informed practices.
  • Prepare detailed, objective, and timely investigative reports and supporting documentation.
  • Collaborate with program leadership, Human Resources, and other departments regarding investigation outcomes and corrective actions.
  • Monitor incident trends, identify root causes, and recommend quality improvement initiatives.
  • Participate in incident review committees and follow-up planning activities.
  • Assist in developing and monitoring corrective action plans to ensure successful implementation.
  • Serve as a subject matter expert on incident reporting systems, documentation standards, and regulatory requirements.
  • Provide training and support to staff on incident management processes and best practices.
  • Maintain professional relationships with regulatory agencies and external stakeholders.
  • Travel to program locations as needed to conduct on-site investigations and reviews.
  • Maintain strict confidentiality and uphold the highest ethical standards in all interactions.

Qualifications Required
  • High School Diploma or GED and extensive experience in incident management, investigations, behavioral health, or human services
  • Valid driver's license with a clean driving record
  • Ability to travel to assigned program locations
  • Ability to pass all required background screenings
  • Strong written and verbal communication skills

    Proficiency with Microsoft Office and electronic documentation systems
Preferred
  • Bachelor's degree in Human Services, Criminal Justice, Social Work, Psychology, or a related field
