Laurelwood Community Living Center is looking for a MDS Coordinator to join our growing team.
MDS Coordinator duties include but are not limited to:- Maintain and update schedules of MDS that are due
- Coordinate the work of the Interdisciplinary Team to complete the MDS, which includes the CAAs and care plan
- Enter or supervise input of MDS into computer systems
- Act as the leader of the MDS and care plan meetings
- Coordinate the completion of the care plan
- Other tasks as required
