Position: Community Support Team- Qualified Professional (Multiple Locations)

We’re recruiting for multiple locations:
Laurinburg, Raeford, Winston-Salem, Gastonia, Lumberton, Fair Bluff, Rockingham, Monroe (NC), and McColl, SC. When you complete your application, please indicate which site you’re interested in so we can route your candidacy appropriately. This is an on-site/community-based role (not remote).

What you’ll do
  • Deliver services:
    Skill‑building, behavioral interventions, psychoeducation, and care coordination that support recovery and daily functioning.
  • Own the plan:
    Co‑create, implement, and review
    PCPs; update goals/interventions based on progress and barriers.
  • Connect resources:
    Link to housing, education, employment, healthcare; support navigation and follow‑through.
  • Use evidence‑based approaches: e.g., behavior modeling/modification, Motivational Interviewing.
  • Guide paraprofessionals:
    Provide direction so interventions are consistent, effective, and aligned to the plan.
  • Assess & document:
    Complete initial/ongoing assessments; enter accurate, timely notes in the EHR (Notenetic preferred)—generally within 24 hours of service and per agency/Medicaid standards.
  • Team & partners:
    Join weekly huddles; liaise with DSS/MCOs, primary care, housing providers, and other partners to keep care seamless.
  • Crisis response:
    Respond promptly; de‑escalate, stabilize, and follow agency protocols.

Must‑have qualifications
  • Meets NC QP criteria:
    • Bachelor’s in Human Services (e.g., Social Work, Psychology, Counseling) and 2+ years’ MH experience with adults; or
    • Master’s in Human Services and 1+ year with the target population.
  • Current/eligible First Aid/CPR, Bloodborne Pathogens, and NCI/CPI Part A (or willingness to obtain upon hire).
  • Valid driver’s license and reliable transportation for local travel.
  • Proficiency with EHRs and productivity tools (Notenetic preferred; Microsoft 365/Google Workspace).
  • Ability to maintain professional boundaries and confidentiality (HIPAA).
Schedule and travel
  • Community‑based role with regular local travel across the service area.
  • Flexibility for evening/weekend coverage as client needs arise.
