• Observe residents and note changes in physical condition, attitude, reactions, appetite, and report changes or unusual findings to charge nurse.
• Transfer residents from bed to chair; chair to bed.
• Turn and position residents.
• Bathe residents – shower and/or tub. Use mechanical lift, transfer patients from chair to tub chair for bathing.
• Feed/assist with feeding residents and/or prepare meal trays for residents; open containers.
• Take vital signs and record. Record fluid intake and output.
• Give oral and dental care to patients.
• Comb patient’s hair.
• Shave patients.
• Provide/Assist residents with incontinence care and/or toileting.
