** Patient Navigation & Care Coordination
• Act as the primary point of contact for orthopedic patients from referral to recovery.
• Coordinate appointments, diagnostics, pre-operative clearances, and post-operative follow-ups.
• Ensure timely movement of patients through the care continuum, minimizing delays.
• Educate patients and families on procedures, expectations, recovery plans, and care pathways.
• Identify and address barriers to care (transportation, financial, social determinants of health).Clinical Pathway Management
• Support implementation and adherence to standardized orthopedic pathways (e.g., total joint replacement programs).
• Monitor patient progress across the continuum and ensure compliance with protocols.
• Facilitate prehabilitation and post-acute care coordination (home health, rehab facilities).Physician & Team Collaboration
• Partner with orthopedic surgeons, advanced practice providers, and multidisciplinary teams.
• Coordinate interdisciplinary communication to ensure seamless care transitions.
• Support physician practices with scheduling optimization and case progression. Patient Experience & Engagement
• Serve as an advocate for patients and families.
• Promote patient engagement tools (education materials, digital platforms, classes such as joint camp).
• Monitor patient satisfaction and address concerns in real time.
Operational & Performance Improvement
• Track key service line metrics (length of stay, surgical volumes, readmissions, case delays).
• Identify opportunities for process improvement and efficiency.
• Participate in quality improvement initiatives and care redesign efforts.
Data & Documentation
• Maintain accurate documentation in the electronic medical record.
• Track patient navigation activities and outcomes.
• Assist with reporting and dashboard monitoring for orthopedic program performance.
Education
• Minimum Requirements --- Graduate of accredited nursing program
Certification/Licensure (including annual requirements)
• Minimum Requirements --- Current NC or Multistate RN license; BSN, BLS, ACLS
• Preferred at Hire --- ONC, must obtain within 1 year of hire Experience (Years/Type)
• Required-----minimum 3 yeears orthopedic or surgical services experience
Other Skills and Competencies
• Minimum Requirements --- Demonstrates excellent communication skills, both written and verbal. Diplomacy, tact, and a positive approach to problem solving are required to be successful. Organizational skills are a must. Fosters a teamwork environment. Must be able to communicate with patients, customers, other departments, and staff in an articulate and professional manner.
