Job Title: Case Manager - Substance Use Disorder (SUD)

Location: Lawrence

Job Type: Full-Time

• Assess client needs through interviews and evaluations to develop tailored case management plans that address both substance use and co-occurring mental health disorders.
• Provide ongoing support and guidance to clients throughout their recovery journey, including crisis intervention, resource navigation, and motivation for behavioral change.
• Collaborate with multidisciplinary teams, including counselors, therapists, medical providers, and community organizations, to coordinate comprehensive care for clients.
• Maintain accurate and up-to-date case files, progress notes, and documentation in accordance with legal and ethical standards.
• Monitor and evaluate client progress, modifying case management plans as necessary to ensure optimal outcomes.
• Facilitate support groups, educational workshops, and community resource sessions to empower clients and promote recovery-oriented practices.
• Advocate for clients' needs and rights, ensuring access to appropriate services, support systems, and community resources.
• Stay current with best practices, treatment modalities, and legal regulations related to substance use disorders.
• Participate in case reviews, team meetings, and ongoing professional development opportunities.

• Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s degree preferred).
• Relevant certification in substance use disorder counseling, case management, or related areas is a plus (e.g., CADC, CAC, or similar).
• Minimum 2 years of experience working with individuals with substance use disorders and/or in case management.
• Strong knowledge of SUD treatment modalities, community resources, and recovery support practices.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work empathetically and effectively with diverse populations.
• Proficient in electronic health record (EHR) systems and case management software.
• Valid driver ’s license and reliable transportation may be required.
