About the Position

Title:

Health Facility Surveyor (HFS) – K0230022

The HFS conducts initial, recertification, complaint, and follow‑up surve y of Medicare/Medicaid certified healthcare facilities for non‑long‑term‑care provider and supplier types (e.g., hospitals, home health agencies, hospice agencies, ambulatory surgery centers, rural health clinics, ESRD facilities, etc.). Surveys determine compliance with CMS conditions of participation or coverage and with Kansas state statutes and regulations requiring licensure. The role is critical for protecting patient safety, ensuring quality of care, and enforcing CMS and state standards.

Position

Summary & Responsibilities

Essential duties include evaluating compliance with federal and state regulations, observing care delivery, reviewing clinical and administrative records, interviewing staff, patients, and families, investigating complaints, documenting findings, and preparing Statements of Deficiencies. The HFS works as part of a multidisciplinary team, participates in pre‑survey planning, entrance and exit conferences, and debriefings. The position requires travel, including overnight stays and off‑hour or weekend work.

  • Conduct onsite surveys of acute and continuing care providers to determine compliance with state licensure and federal certification requirements.
  • Complete required survey forms and make licensure and certification recommendations.
  • Perform observation, record review, and interviews with facility staff, residents/clients/patients, and family members.
  • Conduct surveys individually or as part of a team, including entrance and exit conferences.
  • Interact knowledgeably with healthcare providers, the public, peers, and supervisors regarding state and federal regulations.
  • Maintain professional interpersonal communication skills.
  • Complete documentation as required according to the federal State Operations Manual and submit within required time frames.
  • Compile and document deficiencies according to Principles of Documentation.
  • Organize survey packets and prepare written reports as directed.
Compensation

Hourly Pay Range: $33.55 – $35.70. Salary may vary based on education, experience, or qualifications.

Qualifications
  • Education: High School/GED required.
  • Licensing & Certification: Valid driver’s license; licensed registered nurse with experience in a healthcare facility/service.
