Nurse Practitioner​/Physician Assistant, LMH Health Primary Care- South Iowa

Job Summary

The Nurse Practitioner (APRN) provides direct patient care, leads coordination of care, and applies decision‑making skills based on patient and family healthcare needs in accordance with institutional policies and regulations.

• Develop therapeutic relationships with patients and families, promoting decision making based on assessment of medical histories, symptoms, examinations, diagnoses, conditions and treatments.
• Communicate and collaborate concisely with the clinical team, including physicians, both verbally and in documentation, focusing on patient problems and needs, implementing and monitoring patient care plans, and recording and communicating patient conditions to optimize health outcomes.
• Assess and coordinate patient discharge planning needs with members of the healthcare team.
• Apply collaborative teamwork and leadership as an inter‑professional team member to coordinate patient‑centered care for quality patient and unit outcomes, utilizing information, resources and technology.
• Provide patient‑centered nursing processes, including patient/family involvement, evidence–based practices (EBP), safety and process improvement.
• Exemplify professionalism by applying lifelong learning, contributing to the science of nursing, participating in governance, sharing knowledge to expand best practices, and building ethical, moral and regulatory foundations for patient and unit care.
• Perform other duties in line with the current basic nurse practitioner scope.
• Ensure regular and reliable attendance as an essential function of this position.

• Master's degree in Nursing
• Current Basic Life Support for Healthcare Providers certification
• Current Advanced Practice Registered Nurse license to practice in Kansas

• Prior relevant clinic experience
