A.
Responsibilities and Duties
:
The primary responsibility of the Child &Family Services Intern Case Manager will be case management services, resourceacquisition, crisis intervention, and outreach activities to specified youthand their families. The Intern Casemanager will also assist the CFS Attendant Care/Respite Supervisor withdeveloping, scheduling and overseeing operation of psychosocial/respite groups.

Regular and predictable attendance is an essential function of this position.

Specific duties include:

  • Assist in the development and implementation of the psychosocial/respite groups.
  • Assist with scheduling staff for groups, and manage staff/child ratio.
  • Ensuregroup activities and supplies are ready for group.
  • Ensure that documentation for groups meets medical necessity.
  • Providecase management services, resource acquisition, crisis intervention, and outreach activities. Coordinate these services with other community health and social service agencies.
  • Deliver services in a variety of locations, including but not limited to the client'shome, the school, the Child & Family Services program site, or anothercommunity setting, on an as-needed basis.
  • Providetransportation to and from various activities or appointments as indicated bythe treatment plan or as instructed by the QMHP.
  • Provide services on an as-needed basis both with respect to time-of-day and location.
  • Deliverreimbursable hours of service compliant with established standards. 50% of thejob would be direct services.
  • Complete all paperwork associated withthe financial, clinical, legal, or regulatory management of the case or the Center's operations in a timely and accurate manner.
  • Assist with the evolution of an effective, efficient program of services.
  • Respond tocrises while ?on call? in rotation with other similarly-trained staff.
  • Additional responsibilities may be assigned as circumstances warrant.

B.
Accountability
:
The Child & Family Services Case Manager will be directly accountable to the Child & Family Services Team Leader orhis/her/their designee. The Child &Family Services Team Leader or his/her/their designee will, no less thanannually, complete a performance evaluation in compliance with personnelpolicies.

C.
Qualifications
:
The Child & Family Services Case Manager position requires:

  • BA/BSdegree or four years of equivalent education and/or experience working in thehuman services field;
  • reliable transportation and automobile insurance; and a valid, unrestricted driver'slicense;
  • priorcomputer experience and basic computer fluency;
  • the ability to achieve within one's first six months of employment/practicum aworking competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation ofthe clinical record;
  • the ability to work independently and in cooperation with others;
  • strong organizational skills;
  • strong oral and written communication skills;
  • demonstratedstrong interpersonal skills in working with diverse populations, consumers, and referring agencies;
  • flexibility in assigned working hours;
  • commitment to interdisciplinary and community mental health center approaches; and
  • an energetic and positive approach to the rapidly-evolving changes and challengesof a complex work place.
