Pay Range: $24.77+/hour
Relocation Assistance: Up to $5,000 available

Job Overview

We are seeking a Licensed Bachelor Social Worker (LBSW) to join a hospital-based case management team. This role focuses on patient advocacy, care coordination, discharge planning, and connecting patients and families with appropriate resources to support positive health outcomes.

Responsibilities

• Coordinate patient care plans and healthcare transitions.
• Conduct psychosocial assessments and identify patient needs.
• Collaborate with interdisciplinary healthcare teams.
• Support discharge planning and continuity of care.
• Connect patients with community resources and support services.
• Maintain accurate documentation.

Qualifications

• Bachelor’s Degree in Social Services required
• Current Kansas LBSW License required
• 2+ years of hospital-based social work experience preferred
