Waiver Facilitator

Pay is commensurate with experience within a range of $20.00 to $24.00 per hour. Full time, non-exempt, benefits eligible. The primary responsibility of SED Waiver Facilitator will be to organize and manage the components of the Waiver Program for children that meet the criteria for the targeted population. Specific duties include but not limited to :

• Acting as primary Waiver contact forthe family and other professionals working with the client via the client'sPlan of Care/POC (treatment plan);
• Monitoring the tasks outlined on thePOC with other treatment team members, medication services staff, school andother community providers by communicating with such staff and community providers on a regular basis and coordinating schedules and assignments;
• Coordinating and scheduling respite care in accordance with client's needs;
• Acting as Treatment Plan Coordinator for assigned clients;
• Providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;
• Deliveringservices in a variety of locations, including but not limited to the client'shome, the school, the Child & Family Services program site, or anothercommunity setting, on an as-needed basis;
• Providing transportation to and from various activities or appointments as indicated bythe treatment plan or as instructed by the QMHP;
• Responding to crises while on call in rotation with other similarly trained staff;
• Regular and predictable attendance is an essential function of the job; and
• Additional responsibilities that may be assigned as circumstances warrant.

The SED Waiver Facilitator position requires but not limited to:

• a bachelor's degree in psychology,social work, psychiatric nursing or another related human services field or beequivalently qualified by work experience or a combination of work experience in the human services field and education with one year of experiencesubstituting for one year of education.
• reliable transportation; and a valid,unrestricted driver's license
• prior computer experience and basic computer fluency;
• an ability to work independently and in cooperation with others;
• ability to exercise sound independent judgment;
• flexibility in assigned working hours;
• a commitment to interdisciplinary and community mental health center approaches; and
• an energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.
