Director of Community Geriatrics

The Staff Pad is partnering with a reputable, innovative primary care practice to seek an experienced physician as Director of Community Geriatrics in Boston, MA. In this role, you will deliver direct patient care, provide clinical leadership, and oversee advanced practice providers and senior living programs. You will join a multidisciplinary team committed to moving beyond traditional fee-for-service models to advance value-based, patient-centered care.

Responsibilities

• Deliver comprehensive care to older adults across outpatient, home, and virtual settings
• Manage acute and chronic conditions alongside NPs and multidisciplinary care teams
• Lead annual wellness visits, preventive screenings, and geriatric assessments
• Participate in scheduled on-call coverage
• Provide medical oversight for patients residing in SNFs, ALFs, and LTC facilities
• Conduct regular rounds to evaluate clinical outcomes, quality measures, and care coordination
• Serve as Medical Director for affiliated senior living facilities, supporting quality assurance, policy development, and staff education
• Lead and mentor NPs and APPs through regular chart reviews, joint patient rounds, and ongoing support

Requirements

• MD or DO degree from an accredited medical school
• Board certification or eligibility in Internal Medicine or Family Medicine; subspecialty certification and/or extensive experience in Geriatrics strongly preferred
• Active medical licenses in both Massachusetts and New Hampshire (required)
• DEA license and controlled substance registration in Massachusetts and New Hampshire
• 3+ years of experience in geriatric, long-term care, or complex primary care preferred
• Experience supervising advanced practice providers (APPs) strongly preferred
• Experience with value-based care programs, including Medicare's Advanced Primary Care Management Program
• Strong attention to accurate coding and clinical documentation
• Patient-centered, proactive, and team-oriented approach
• Strong communication, leadership, and interdisciplinary collaboration skills
