Requirements:

• Active license as a Physical Therapist in the applicable state.
• Bachelor's degree required;
  Master's or Doctorate preferred.
• Basic Life Support (BLS) certification preferred.
• Prior experience in a long-term care or skilled nursing facility preferred.

Roles & Responsibilities:

• Evaluate residents and establish a therapeutic plan of care with functional goals.
• Provide consultative support to the healthcare team regarding residents' plan of care and discharge needs.
• Document all interactions with residents in the Electronic Medical Record accurately.
• Collaborate with other therapy professionals to ensure optimal resident care.
• Recommend and educate residents on the use of assistive devices.
• Supervise assistant-level staff as appropriate and ensure adherence to professional standards.
