Requirements:
- Active license as a Physical Therapist in the applicable state.
- Bachelor's degree required;
Master's or Doctorate preferred. - Basic Life Support (BLS) certification preferred.
- Prior experience in a long-term care or skilled nursing facility preferred.
Roles & Responsibilities:
- Evaluate residents and establish a therapeutic plan of care with functional goals.
- Provide consultative support to the healthcare team regarding residents' plan of care and discharge needs.
- Document all interactions with residents in the Electronic Medical Record accurately.
- Collaborate with other therapy professionals to ensure optimal resident care.
- Recommend and educate residents on the use of assistive devices.
- Supervise assistant-level staff as appropriate and ensure adherence to professional standards.
