Care Source seeks a Community Health Worker for Massachusetts (Suffolk / Norfolk / Surrounding Areas) to support enrollees with social determinants of health and access to care. You will conduct in-person or telehealth visits, help build holistic care plans, and document interactions in the CER with care team collaboration.
The role requires driving, meeting complex needs of members, and coordinating with PCPs and behavioral health resources. Flu vaccination and license requirements apply.
