RN Care Manager:
Monday - Friday (HYBRID)

Outpatient setting

Seeking an RN with 2 or more years clinical experience. Prefer experience with Care Coordination, Case Management, Discharge Planning, Utilization Review, etc.

Active MA RN License. Certified Case Manager (CCM) preferred. Driver's license in good standing.
