Registered Nurse – Hospice

Hospice Nurse

Provides comprehensive home hospice nursing care by assessing patient needs, developing care plans, and coordinating services in accordance with physicians' orders and agency policies. Responsible for ensuring quality patient outcomes and supervising home health aides and licensed practical nurses.

Responsibilities:

• Coordinate and deliver patient-centered hospice care in home settings following clinical protocols.
• Evaluate patient conditions through assessments and modify care plans as needed.
• Supervise and evaluate the performance of home health aides and licensed nurses.
• Document patient interactions, assessments, and interventions accurately in the EMR system.
• Communicate effectively with physicians, families, and interdisciplinary team members.
• Travel up to 75 miles within the designated territory to provide care in patients' homes.

Qualifications:

• Hold an active RN license in the state of practice.
• Possess at least 1 year of recent adult home health or hospice experience.
• Demonstrate proficiency in Home Care Home Base (HCHB) with recent usage within the past 6 months.
• Be proficient in Oasis documentation if required.
• Possess a valid driver's license, reliable transportation, and agency-required liability insurance.
• Ability to travel up to 75 miles from the home clinic location.
