Description:

Provides, coordinates, and directs the provision of home nursing care according to physician's orders.

Nursing care is based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.

Degree of travel includes home visits and periodic office meetings.

Must have reliable transportation and a current driver’s license.

Agency-required liability insurance is necessary.

Average number of patients per day is 5-8.Contractor will be assigned between 5-10 patients per day.

Each visit and activity (documentation, travel, etc.) holds a different point value.

Guaranteed hours are not provided.

Floating to a nearby facility may be discussed if there is low patient census.

Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.

First-timers are not accepted; candidates must have recent specialty experience.

Weekend requirements vary by location and profession.

On-call requirements may vary by location and profession.

Travel up to 75 miles from the home clinic is required.

Mileage will be reimbursed at the IRS standard rate.

Local candidates are accepted at the same rate.

Pending licenses are not accepted; candidates must have a license in hand.

All professionals must be proficient in Home Care Home Base and have used it within the last 6 months.

If not, a retraining course will be required.

Documentation must be completed within 24 hours after the appointment with the patient.

All travelers must be proficient in relevant charting systems.

There are no guaranteed hours with this contract.

The clinician must hit weekly productivity points equivalent to 40 hours per week.

All overtime must be pre-approved.

If not receiving enough patients each week, there will be an opportunity to float to another nearby clinic.
