Human Services Coordinator I

Lawrence, MA
The Department of Developmental Services (DDS) offers a full‑time, 37.5‑hour Human Services Coordinator I (HSC I) position in the Merrimack Valley Area Office. The coordinator is the primary contact for individuals and families served by DDS and is responsible for building respectful and collaborative partnerships with families, guardians, provider agencies and other stakeholders.
Responsibilities- Coordinate and facilitate development of Individual Service Plans (ISP), Life Plans, and Individual Transition Plans (ITP) for assigned caseloads.
- Monitor implementation of services provided by DDS‑funded residential, day and support program providers.
- Arrange, organize, and coordinate DDS‑funded and generic support services in response to each individual's needs.
- Maintain communication and outreach with community services and other state agencies.
- Support specialized population groups (children, transitioning young adults, adults, individuals with autism, and self‑direction participants) as outlined in the role.
- Monitor safety and well‑being of assigned individuals.
- Provide advocacy in human, civil, and legal rights.
- Facilitate team meetings; monitor follow‑through on team recommendations and maintain relevant records and databases.
- Maintain regular and consistent contact with individuals, service providers and families.
- Arrange crisis intervention as needed and recommend follow‑up planning.
- At least three (3) years of full‑time or equivalent part‑time professional experience in human services or social work, or an equivalent combination of education and experience.
- Bachelor's degree or higher in a related field may substitute for two (2) years of the required experience.
- Valid driver’s license if motor vehicle travel is required.
- For certain assignments, a Bachelor's degree in social work, psychology, sociology, counseling, or related fields is required.
- Strong communication, organization, and problem‑solving skills.
