Position

Summary

Casa Esperanza, Inc. is seeking a Housing/Homeless Case Manager for our Lawrence, Massachusetts location. The Housing/Homeless Case Manager will provide intensive supportive services to chronically homeless adults living with co-occurring substance use and mental health disorders (CODs) and help them develop skills to build meaningful, purposeful lives in the community through housing and case management. Our program assists chronically homeless adults achieve their goals related to safety, stability and recovery.

The Housing/Homeless Case Manager is embedded into clinical teams and works closely with other staff to provide comprehensive, coordinated care. S/he will engage clients where they are to help them develop and build the basic skills they need to obtain permanent housing.

Responsibilities
  • Collect all necessary documentation for HUD eligibility
  • Conduct social service needs assessment, individualized service plan and care coordination activities with clients
  • Identify and recommend supportive services that will assist participants in reaching their goals, provide referrals as needed
  • Specific areas to address with participants include workforce development, employment placement and vocational skills development, post-placement job retention, early childhood education, and health services
  • Maintain regular contact with participants (monthly at minimum) to review progress and reassess service plan
  • Update and report on program census on a weekly basis
  • Collaborate with the MA Department of Public Health, Bureau of Substance Abuse Services, and Department of Mental Health to facilitate program development and implementation
  • Provide support and technical assistance to clients and programs throughout the intake process, including forms required by participating state agencies and treatment vendors
  • Provide outreach to introduce residents to case management program and educate residents about services available to them (including in-person, special events, and mailings)
  • Provide culturally- and linguistically-appropriate supportive services to participants
  • Maintain accurate records, data and reporting as required by the program
  • Meet with supervisor on a biweekly basis
Qualifications / Requirements
  • Previous case manager experience in the field of addiction disorders as well as with co-occurring mental health and trauma issues
  • Experience in community outreach and case management activities
  • Master's degree in social work, psychology, human services, or related field
  • 3-5 years' experience in the field of human services, case management, family counseling, vocational, and/or substance use issues
  • Ability to work effectively with diverse cultural groups
  • Good organizational and communication skills, both oral and written
  • Hold a driver's license and must have a car
  • Bilingual (Spanish/English) required
  • Knowledge of MA housing law and Reasonable Accommodations preferred
