Age Span is seeking a Congregate Housing Coordinator-Care Manager to work within an interdisciplinary team to help older adults live with dignity and independence. You will coordinate housing services, lead the assessment team, and provide referrals to community resources while maintaining meticulous documentation.
The role requires a BA/BS in social work or related field, plus strong communication and computer skills. A driver’s license and possible bilingual Spanish ability are preferred.
