View Point Health's Court Services provides group, family, and individual education, skills‑based training and supportive counseling to consumers and their families/significant others. Designs counseling/education services that meet the individual needs of the consumer. Educates families/significant others on the addiction and recovery process. Assesses consumers treatment needs and assists him/her to develop appropriate individual treatment goals.
Duties & Responsibilities- Provides professional therapeutic services to persons with mental illness and/or addictive disease.
- Arranges appointment schedule at optimal intervals to maximize use of work time.
- Conducts face‑to‑face comprehensive assessment to identify problems and service needs.
- Writes individual service plans that meet required standards for diagnoses, content, justifications for treatment, evidence of consumer input, and treatment time frames.
- Writes service plans that are clear, concise, complete, grammatically correct, timely, and properly coded.
- Secures or provides necessary signatures required to secure authorization for services.
- Follows all regulations related to the credentials required to provide authorized services.
- Provides individual, family or group therapeutic counseling and/or educational and skill development sessions as indicated in the service plan.
- Monitors consumer progress and compliance with the service plan.
- Writes timely progress notes that relate directly to the treatment outcomes described in the service plan.
- Assembles, analyzes, and interprets new/updated information about the consumer in order to reassess treatment needs.
- Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated.
- Provides crisis intervention services to existing consumers as needed.
- Serves as case manager to facilitate and ensure communication, linkage, and continuity of care between consumers, physicians, other organizational components, community resources and service providers.
- Establishes linkages and coordination of services with community agencies.
- Develops and maintains cooperative working relationships with community service providers.
- Informs consumers and families of the availability of community advocacy and support groups.
- Provides group, individual and family sessions for court participants.
- Provides documentation on all services provided in View Point Health medical record and court case management system.
- Provides case management services and linkage to court participants.
- Functions as part of the Drug, DUI & FTC Court teams.
- Attends court and staffing for Drug, DUI & FTC Court as needed.
- Attends training as needed for court evidence‑based curriculums.
- Completes GPRA Assessments for SAMHSA grant and enters into SPARS system.
- Keeps spreadsheets for GPRA Assessments.
- Other related duties as assigned
- Master's degree in social work and counseling from an accredited college or university and eligibility to be licensed.
Note:
Some positions may require licensure in the state of Georgia as an LMSW, LAPC, LCSW, LPC or LMFT
- Certification as an addiction counselor (CAC I/II, CADC I/II, MAC, CAADC, etc.)
- Experience providing case management services.
- Experience with Excel and/or spreadsheets.
- Experience providing groups.
- Experience working with clients involved in the criminal justice system.
- Experience working with co‑occurring populations.
- Experience working in an accountability court.
- Must be able to lift 20 pounds.
- Requires long periods of sitting/standing.
- Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
