The Program Manager
- Adult MH Supportive Housing provides general oversight and direction to all or varying aspects of the Residential Program in Gwinnett, Rockdale, Newton and Clayton counties to include:
Intensive Residential Treatment and Group Homes, Semi-Independent and Independent Level Placements, as well as Supportive Housing expansion programs. The Program Manager may complete comprehensive mental health and addictive disease assessments and/or psychosocial assessments and related referral and enrollment processes for individuals seeking supportive housing services throughout Gwinnett/Rockdale/Newton/Clayton areas as needed. The Program Manager monitors and operates the program using state and federal program guidelines to include the guidelines of DBHDD, HUD, CARF, DCA and other regulatory authorities.
The job requires flexibility in working hours/days and locations (Gwinnett, Rockdale, Newton, Clayton) to ensure shift coverage and smooth program operations.
Duties & Responsibilities:
- Supervises a team of professionals responsible for providing Residential Services and other Support Services.
- Guides, instructs, and supports team in their work assignments.
- Serves as a primary contact and resource to residential support and/or clinical program staff for administrative information and technical assistance.
- Maintains databases, logs, inspection reports, supervision documents and other records of routine information.
- Monitors supplies and orders replenishments.
- Maintains residential Policy Manuals and Standard Operating Procedures and updates as needed.
- Assists with administrative or clinical meetings.
- Works closely with staff to encourage treatment and promote safe and stable housing for individuals served.
- Process referrals, screenings, and completes brief case management as needed.
- Provides training regarding Rights of individuals served, crisis response, site audit preparation, etc.
- Participates with the individual in preparing an initial and/or ongoing individual service plan that utilizes assessment data, diagnostic formulations, identified strengths, needs, preferences and resources to develop appropriate goals and objectives as needed.
- Provides triage and crisis intervention services to individuals presenting in need.
- Performs re-authorizations and related assessments as required by state/program guidelines as needed.
- Provide recommendations for appropriate individual residential placements based on eligibility and level of need.
- Supervises, coordinates and directs the social service activities of the program. Provide person(s), families or vulnerable populations with the psychosocial support needed to cope with chronic, acute or terminal illnesses, mental, physical or developmental disabilities. Makes referrals for other services as needed.
- Completes grant renewal applications and assists with Program contracts as needed.
- Monitors residential budget and supervises billing activities to ensure completion for adequate program revenue and to meet state/program guidelines.

- Monitors program bed utilization to ensure vacancies are filled timely and as required by state/program guidelines.
- Maintain existing and pursue additional community partnerships and vendor relationships to support the residential program.
