- Promote, coordinate, and facilitate evidence-based programs, educational sessions, and workshops.
- Provide information and education on health, wellness, prevention, and available community resources.
- Conduct outreach to individuals, families, and community partners to increase awareness and participation in programs.
- Assist with community events, health fairs, presentations, and other outreach activities.
- Conduct client intake, assessments, and needs screenings.
- Provide ongoing case management and follow-up to help clients achieve their goals.
- Connect individuals and families with healthcare, social services, housing, food assistance, behavioral health, employment, education, and other community resources.
- Assist clients in navigating health and social service systems.
- Maintain accurate and timely client documentation, case notes, referrals, and program records.
- Follow up with clients to assess progress, address barriers, and ensure appropriate connections to services.
- Maintain confidentiality and follow organizational policies and applicable privacy requirements.
- Support the planning and implementation of youth programs, activities, workshops, and educational opportunities.
- Develop positive, trusting relationships with youth and families.
- Encourage youth participation, leadership, healthy decision-making, and community engagement.
- Assist with recreational, educational, and skill-building activities.
- Communicate with parents, guardians, schools, and community partners when appropriate.
- Serve as a welcoming and professional point of contact for clients, families, and community members.
- Respond to questions, concerns, and requests in a timely and respectful manner.
- Provide culturally and linguistically appropriate customer service in English and Spanish.
- Build positive relationships with community organizations, healthcare providers, schools, and other partner agencies.
- Advocate for clients and help reduce barriers to accessing services.
- Complete required documentation, reports, data entry, and program tracking.
- Assist with monitoring program outcomes and client participation.
- Participate in staff meetings, trainings, and professional development activities.
- Support other nonprofit programs and organizational initiatives as needed.
- Perform other duties as assigned.
- Bachelor's degree in Psychology, Social Work, Human Services, Community Health, education, or a related field preferred.
- 3-5 years of Experience working with diverse communities, families, youth, or underserved populations.
- Experience providing workshops / programs preferred.
- Experience working in a NPO is a plus.
- Advanced computer knowledge: MS Office, Salesforce is a plus.
- Customer service oriented.
- Valid driver’s license. Willing to commute to all Ser Familia’s locations as needed.
- Fully bilingual in English and Spanish, both spoken and written.
