DEFINITION

The Medical Assistant (MA) Care Coordinator is a clinical MA leadership position responsible for training and education of all physician and clinical support staff for a large multispecialty clinic group. Care Coordination duties include performing periodically as an electronic health record clinical trainer. Training requirements are designed to optimize the delivery of healthcare within each practice, focusing on training clinic staff to perform value based activities related to patient and family engagement, patient safety, care coordination, population health, the efficient use of healthcare resources and clinic process/effectiveness.

Works with incumbent clinic staff to optimize clinical practice in support of physicians i.e., writing referrals and scripts, reviewing lab results, etc. Training may be performed on an individual basis or in a classroom setting.

This position is primarily responsible for ensuring all staff utilizes the electronic medical record appropriately, ensuring maximum reimbursement for such programs as MIPs and the ACO. As a member of the practice management team, is responsible for development and implementation of new workflows to improve clinic practice and maximize reimbursement. Works as part of the Practice Management team on special projects such as Joint Commission readiness, clinical process improvement programs utilizing data to drive change and other special projects as assigned.

Is also required to float to a specialty clinic to work as a staff nurse when needed.

Is responsible for practicing within the MA scope of practice, managing any assigned personnel and promoting collaboration among the disciplines. Works independently requiring minimal supervision in order to succeed. Patient population includes: all age groups to include children, adolescents, adults and geriatrics. Additional duties may include oversight of joint commission readiness in various clinics through auditing and re-education and performing care coordination of referral services.

QUALIFICATIONS Clinical Experience
  • One to two years of physicians' office nursing experience.
  • Familiarity with various software programs including but not limited to electronic health records, word, excel, power point.
  • Ability to speak in front of groups and teach in a classroom setting.
