What You'll Do

As a Home Health Registered Nurse, you'll provide skilled nursing care while working independently and collaborating with physicians, therapists, caregivers, and other healthcare professionals.

Responsibilities include:
  • Perform Start of Care (SOC), OASIS Recertifications, Resumption of Care, and Discharge assessments
  • Conduct Skilled Nursing Visits (SNVs)
  • Administer medications and treatments as ordered
  • Assess wounds and monitor healing progress
  • Educate patients and family members on disease management and home care
  • Coordinate care with physicians, Physical Therapists, Occupational Therapists, Speech Therapists, and other healthcare providers
  • Develop meaningful relationships with patients while helping them remain safely in their homes
Qualifications:
  • Active Oklahoma Registered Nurse (RN) License
  • Current driver's license and reliable transportation
  • Ability to travel between patient homes
  • Strong assessment, communication, and documentation skills
