What You'll DoAs a Home Health Registered Nurse, you'll provide skilled nursing care while working independently and collaborating with physicians, therapists, caregivers, and other healthcare professionals.

- Perform Start of Care (SOC), OASIS Recertifications, Resumption of Care, and Discharge assessments
- Conduct Skilled Nursing Visits (SNVs)
- Administer medications and treatments as ordered
- Assess wounds and monitor healing progress
- Educate patients and family members on disease management and home care
- Coordinate care with physicians, Physical Therapists, Occupational Therapists, Speech Therapists, and other healthcare providers
- Develop meaningful relationships with patients while helping them remain safely in their homes
- Active Oklahoma Registered Nurse (RN) License
- Current driver's license and reliable transportation
- Ability to travel between patient homes
- Strong assessment, communication, and documentation skills
