Responsibilities
  • Complete individual, family, and group outreach to clients as assigned to include the following:
    • Teaching skills including but not limited to coping skills, decision‑making skills, anger management skills, life skills, conflict resolution skills, communication skills, and independent living skills.
    • Provide case management services, information, education, and referral services.
  • Provide services to CARS clients as assigned, completing the following:
    • An initial referral conference at intake including the client, family, and OJA worker
    • A monthly report for each CARS client
    • Maintain contact with OJA and Juvenile Bureau staff in regard to each client referred
    • Be on call to clients through either an agency cell phone or a second number downloaded on your personal phone
  • Provide mentoring services to clients as assigned which include but are not limited to:
    • Completing initial assessment and follow up assessments every three months
    • Providing assistance to youth with educational problems and deficiencies
    • Advocating on behalf of the youth with school personnel, parents, community service sites, etc.
    • Teaching life skills.
    • Assisting age‑appropriate youth in seeking and obtaining employment.
    • Encouraging youth to attend counseling appointments, if applicable, and maintaining communication with the client’s counselor and other members of the treatment team.
    • Assisting youth in participating in appropriate leisure activities to improve social skills and increase positive peer interaction.
    • Acting as a role model for youth while engaging them in community activities.
    • Meeting with OJA Unit staff to ensure effective communication for those youth on probation, if applicable.
    • Assist youth in completing community service necessary for completion of probation, if applicable.
  • Provide weekly services to each client on caseload.
  • Make contact with each new client within five days of the referral and document this in Chart Caddy.
  • Complete assessments on each client, including thorough completion of each intake document and assessment tool.
  • Complete treatment plan development, set goals and objectives for each client in the client’s own words, according to established time frames, assuring that all appropriate information is included and shared with treatment team members. Assist with ensuring that each client and/or parent/guardian is offered a copy of the treatment plan goals and objectives.
  • Complete case management/individual outreach and discharge planning components of the client’s treatment plan and reviews, documenting discussion of the plan with the client, case worker, or family as appropriate.
  • Complete progress notes in Chart Caddy after each scheduled session, including cancellations, no‑shows, and completed sessions. Also, complete information‑only notes in Chart Caddy for each client who was not able to be scheduled that week.
  • Turn in weekly billing by the assigned deadline each week.
  • Complete progress notes and turn in verifications by the assigned deadline each week.
  • Complete treatment plan reviews at six‑month intervals, including completion of necessary assessments which were initially completed at intake.
  • Develop appropriate aftercare plans with the client, assessing the need, and arranging for community support as appropriate and documenting these plans.
  • Contact each client after a no‑show to determine if the client is still interested in services and remind the client of the no‑show policy.
  • Complete corrections on all case files, progress notes, and treatment plans as provided by the Outreach Program Supervisor within a timely manner.
  • Complete prevention courses as assigned which could include Parenting, Anger Management, Botvin Life Skills, Camps, or First Time Offender Classes.

    Duties include:
    • Completing a sign‑in sheet for each class completed
    • Complete pre‑ and post‑tests and surveys for all courses
    • Teach curriculum provided for each course
  • Maintain communication with Outreach Program Supervisor by reading and responding to emails daily.
  • Participate in supervision at least twice per month with Outreach Program Supervisor.
