Job Summary
The registered nurse plans, organizes, and directs/coordinates home care services, with emphasis on community health education. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.
Duties & Responsibilities- Develop and initiate the plan of care; assume responsibility for ongoing interdisciplinary assessment and individualized planning in partnership with patient, representative and caregiver(s).
- Initiate preventive and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care.
- Provide counseling and education to patients, caregivers and families.
- Prepare clinical notes and communicate with the primary physician and other health care practitioners regarding the current home health plan of care.
- Identify discharge planning needs and implement them prior to patient discharge.
- Communicate with community health personnel to coordinate the care plan.
- Participate in the HHA’s quality assessment and performance improvement program and in-service training.
- Participate in on‑call duties as defined by the on‑call policy.
- Ensure arrangements for necessary equipment and services are available.
- Instruct, supervise and evaluate home health aide care provided every two weeks.
- Supervise LPNs/VNAs and/or HHAs/CNAs as directed.
- Graduate from an approved nursing school and hold a valid state nursing license.
- Minimum two years of experience; at least one year in public health or home care nursing preferred.
- Maintain current CPR certification.
- Valid driver’s license and ability to drive or have access to reliable transportation for agency‑related travel.
- Self‑directed, able to work with minimal supervision.
- Excellent observation, problem‑solving, verbal and written communication skills.
- Organize and prioritize workload independently; management experience not required.
- Responsible for supervising home health aides.
- Physical ability to walk, stand, lift, and transfer patients; lift up to 50 lbs; perform considerable reaching, stooping, bending, kneeling, and crouching.
Physical Requirements
- Exposure to blood and bodily fluids, latex, chemicals, disease‑hazard drugs, TB (mask required), mechanical/electrical hazards.
- Physical demands include prolonged walking or standing, lifting, carrying and moving objects up to 100 lbs, bending, kneeling, squatting, moving the upper body above or below shoulder level, and repetitive hand movements.
- Requires stable posture and gait to perform patient care and routine tasks; may work extended hours when necessary.
