Job Summary

The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to an assigned resident under the supervision of a registered nurse (RN). Delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Operating as a member of the nursing team, the NA follows infection prevention and control policies and uses Personal Protective Equipment as required.

Provides assistance with basic health care needs including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to the resident. Assists with transferring, repositioning, and walking using correct techniques, and may provide range of motion and passive exercises. Assists with food preparation and dining to create a positive dining experience. Supports residents with meals and snacks and provides water and supplements as appropriate.

The NA also aids providers and nursing staff with procedures as needed. Documents resident interactions and outcomes related to care, behavior, activity, and dietary. Communicates changes in condition and care concerns to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to maintain clean and functional living areas. Assists with wellness and recreational programs and encourages resident participation.

May participate in departmental or organizational committees as needed.

Qualifications
  • High school graduate or general equivalency diploma (GED) preferred, but not required.
  • Minimum age of 16; prior clinical or nursing assistant experience in long-term care preferred.
  • Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state-certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.
  • Satisfactory completion of yearly department-specific competency testing and maintenance of continuing education credits (minimum 12 hours annually). Maintain all department-specific required certifications.
  • For applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry when applicable.
  • Dependent on facility and responsibilities, may be classified as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license; must be medically qualified to perform essential driving functions per Sanford Driving Policy.
