Assignment Duration:
This is a 13-week contract position.
Schedule:
Day shifts (Monday - Friday) with one in four call rotation for nights, weekends, and holidays.
Job Description:
The Home Health Registered Nurse (RN) will be responsible for providing care to patients in their own homes. The role involves conducting OASIS assessments and creating care plans tailored to individual patient needs.
Responsibilities:
- Conduct OASIS assessments for in-home patients.
- Develop and implement individualized care plans.
- Participate in a call rotation for nights, weekends, and holidays, with a rotation of one in four.
Required Experience / Certifications / Licensure:
- Minimum of 2 years of experience performing OASIS assessments and care planning for in-home patients.
- Previous experience with Epic software is a plus.
- Valid RN license in the state of Iowa.
