Registered Nurse – Home Health

In-Home Nursing Care Position

Provide in-home nursing care to patients, conducting assessments, care planning, and delivering treatment in a domiciliary setting. Ensure patient needs are met with compassionate and professional support.

Responsibilities:

• Perform OASIS assessments to evaluate patient conditions and determine appropriate care plans.
• Develop and revise personalized care plans based on patient needs and clinical findings.
• Administer medications and treatments following established protocols and physician orders.
• Collaborate with the healthcare team to coordinate patient care and ensure continuity of services.
• Participate in call rotation, including nights, weekends, and holidays, to provide on-call support.
• Document patient interactions and care activities thoroughly using electronic medical records.

Qualifications:

• Hold an active RN license in Iowa or the applicable state.
• Have at least 2 years of experience performing OASIS assessments and in-home care.
• Demonstrate proficiency with care planning and clinical documentation.
• Previous experience with Epic EHR system is a plus.
• Possess excellent communication and organizational skills.
• Be willing to participate in a call rotation schedule during non-standard hours.
