LVN Charge Nurse

Touchstone Communities is seeking a compassionate LVN Charge Nurse to join our family-oriented senior care team. In this role, you'll coordinate and deliver resident-centered nursing care, administer medications and treatments, and supervise nursing assistants to ensure high-quality clinical outcomes. You'll collaborate closely with physicians, families, and interdisciplinary team members to support individualized care plans. Responsibilities

• Provide direct nursing care to residents in a long-term care setting
• Serve as charge nurse, supervising CNAs and support staff on assigned shift
• Administer medications and treatments per provider orders
• Monitor resident conditions and respond promptly to changes in status
• Document assessments, interventions, and outcomes accurately and timely
• Collaborate with physicians, therapists, and families on care plans
• Ensure compliance with facility policies, state, and federal regulations
• Support and mentor staff to promote best practices and resident safety
• Participate in care conferences and quality improvement initiatives
• Promote a compassionate, resident-centered, family-oriented environment
