The Pharmacist provides comprehensive pharmaceutical care to inpatients by ensuring safe, effective, and evidence-based medication use. This pharmacist's role involves collaborating closely with physicians, nurses, and interdisciplinary teams to optimize therapeutic outcomes, prevent medication errors, and support informed clinical decision-making. The Hospital Pharmacist also plays a key role in sterile compounding, medication reconciliation, and compliance with hospital and regulatory standards.

Responsibilities- Clinical Medication Review & Verification:
Evaluate, verify, and optimize inpatient medication orders for safety, accuracy, and clinical appropriateness. - Interdisciplinary Collaboration & Rounding:
Participate in patient care rounds and partner with physicians, nurses, and care teams to support evidence-based medication therapy decisions. - Medication Preparation & Distribution Oversight:
Ensure accurate dispensing and oversee sterile and non-sterile compounding processes in compliance with USP 797/800 standards. - Medication Reconciliation & Transitions of Care:
Perform medication reconciliation during admission, transfer, and discharge to prevent errors and support continuity of care. - Patient & Staff
Education:
Provide medication education to patients when needed and serve as a drug-information resource for clinicians and hospital staff. - Quality, Safety & Compliance:
Support medication safety initiatives, report adverse drug events, maintain regulatory compliance, and uphold hospital policies/protocols. - Inventory & Operational Support:
Manage medication inventory, emergency supply readiness, and support efficient pharmacy operations. - Clinical Program Participation:
Contribute to antimicrobial stewardship, pain management, or other hospital clinical initiatives as assigned.

- Current state:
Registered Pharmacist License. - Experience in hospital pharmacy operations is preferred.
- A B.S. degree in Pharmacy is required; a Doctor of Pharmacy degree is preferred.
- BLS and ACLS certification preferred.
