The Guidance Center (TGC) is currently seeking an Adult
Targeted Case Manager to be the newest member of our innovative interdisciplinary team. This position provides Community Psychiatric Support and Treatment (CPST) and Targeted Case Management (TCM) services to SPMIand SMIadults,following the strengths model of case management.

Evenings Required (1+ days per week after 5 pm).

Competitive hourly rate of $26.11 to $28.51; depending upon qualifications.

Primary Duties

Under the supervision of a QMHP, the TCM willfacilitatethe treatment plan process by inviting all involved participants, to include client to develop or update a plan which addressesidentifiesneeds, defines goals and objectives, and action steps which support community living and participation.

Enter treatment plan details into electronicrecordsand other document sin a timely manner.

Monitor treatment plans to ensure the plan is current, ensure that updates areaccomplishedbefore the plan expires,and enlistassistanceof supervisor if problems are encountered.

Assistin onboarding and care coordination for the Service Members Veterans and Families care team, as needed

Provide face-to-face, phone and written communication with caregivers, other service agencies, housing resources, and medical services toassistwith meeting client/family needs.

Provide assertive outreach directed toward engaging andmaintainingclient relationships as well as relationships with caregivers

Assure that client information is communicated to key staff prior to and after appointments

Advocate for and with clients as theyseekto achieve their goals and overcome barriers

Assist clients or caregivers in obtaining access to needed medical, social, educational, and other services.

Transport clients to and from appointments and activities when necessary.

QUALIFICATIONS EDUCATION AND EXPERIENCE

Bachelor’s degree required or be equivalently qualified by work experience or a combination of education andwork experience in the human services field with one year of experience substituting for one year of education.
