In-Home Health Clinician - Nurse Practitioner or Physician Assistant Wenatchee

Job Summary

Signify Health, part of CVS Health®, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In‑Home Health Evaluations. The clinician will visit patients in their homes, perform medication and medical history reviews, conduct physical examinations, and administer diagnostic tests as ordered by the patient’s health plan.

• Conduct 45‑minute in‑home visits, providing personalized health evaluations.
• Review medication history, assess medical status, and perform physical examinations.
• Order and administer diagnostic & preventive services such as spirometry, diabetic eye exams, and bone density screening, when indicated by the health plan.
• Use an iPad and a clinical workflow application for all evaluations.
• Travel locally and within licensed states as required.
• Conduct virtual visits when needed.
• Provide patient education and support to help individuals improve their health.

• Active, unrestricted license(s) in the coverage area.
• Board certification required.
• Multi‑state licenses (preferred).
• Experience in primary care or preventive health services.
• Strong communication and interpersonal skills.

Wenatchee and surrounding area.

Anticipated weekly hours: 40. Full‑time. Pay range: $95,738.00 – $ (base salary or hourly rate). Actual offer based on experience, education, geography, and other factors.
