Title:
Physical Therapist Assistant (PTA) SNF / Powerback Rehab Days
Location:
Lebanon, NH 03766, skilled nursing facility with a strong rehab program (Lebanon Center, Genesis).
Dates: 04/13/26 07/11/26 (13 week contract)
Schedule:
8:00 AM 4:30 PM, 30 minute unpaid meal (8 hour days)
Days:
Sunday through Saturday (schedule set by rehab director; up to 5 shifts/week)
Guaranteed hours: 35 hours/week
Status:
Open, accepting submissions
Gross Blended Rate: $34 $40
Core PTA responsibilities Deliver physical therapy interventions under the direction and supervision of a licensed PT for post acute, ortho, neuro, and medically complex SNF residents.
Implement and progress treatment plans focused on gait training, transfers, balance, strengthening, and functional mobility to support safe discharge or long term function.
Assist with evaluations (data collection, objective measures), update daily notes, and document treatments and resident response in the EMR per regulatory and facility standards.
Participate in IDT meetings as requested, collaborate with nursing and OT/SLP, and provide caregiver and patient education regarding exercises and safety.
Qualifications Active New Hampshire PTA license in good standing.
Graduation from an accredited Physical Therapist Assistant program.
SNF / short term rehab experience strongly preferred; comfortable managing a mixed post acute and long term care caseload.
