Travel RN - Case Management​/Utilization Review - Case Management

American Traveler is seeking an experienced RN Care Manager with trauma setting experience and certification for a weekday, inpatient case management position.

Job Details

• Work in inpatient units within an acute care hospital,

• Weekday schedule:
Monday through Friday, 8:00am to 4:40pm,

• No weekend or on-call responsibilities,

• Caseload management of diverse inpatient care needs,

• Attend interdisciplinary rounds and collaborate on care planning,

• Utilize Epic EMR for patient documentation and orders,

Job Requirements

• Active NH or compact RN license required,

• Certification as a care manager required,

• Bachelor of Science in Nursing (BSN) required,

• Minimum 3 years of inpatient clinical experience,

• At least 1 year of care management experience required,

• Recent Level 1 or comparable Level 2 trauma experience within the last 2 years required,

• Current Basic Life Support (BLS) certification is mandatory,

• Candidates must reside outside a 75-mile radius-no local candidates allowed,

Additional Information

• Responsibilities include initial assessments, developing and facilitating care plans, and collaborating with patients, families, and healthcare teams,

• Frequent communication with medical staff and post-acute referral sources to coordinate discharge needs such as home health and inpatient rehab,

• Place and document orders for home health, DME, and nutrition in Epic,

• Attend interdisciplinary rounds as part of the care coordination process,
