Description As an RN Case Manager at Visiting Nurse and Hospice, you will play a crucial role in patient care. You will work independently while being part of a dedicated team, ensuring a balance between work and life. In this role, you will develop and manage care plans, coordinate with interdisciplinary teams, and provide ongoing patient assessments. Your responsibilities will include teaching self-care techniques and ensuring communication between patients and healthcare providers.
