Behavioral Healthcare Provider ; LISW​/LPCC​/IMFT

Overview

Behavioral Healthcare Provider 2 (LISW/LPCC/IMFT) – Position :  (260003VQ). Full‑time, salary: $68,000 annually.

Location:

Lebanon Correctional Institute, Lebanon, OH. Union: 1199.

• Complete detailed mental health evaluations, crisis services, and suicide watch assessments within scope of practice.
• Provide clinical direction to lower level workers and may serve as treatment team coordinator.
• Conduct advanced practice individual and group clinical interventions as prescribed on individual treatment plans, directed by supervisor.
• Provide clinical case management for assigned caseload, including monitoring offender compliance with treatment plans.
• Complete psychosocial needs and treatment assessments as part of interdisciplinary treatment planning.
• Consult with institution staff regarding inmates’ institutional adjustment and service needs.
• Initiate and maintain contact with families and community agencies.
• Seek and provide information regarding patient care and treatment.
• Make appropriate community and re‑entry referrals, per mental health treatment team.
• Write reports and documentation: inmate contacts, progress notes, discharge plans, and other required forms.
• Attend supervision meetings, in‑service training, workshops, and peer review sessions.

Licensed or eligible to obtain one of the following Ohio licenses:

• Licensed Independent Social Worker (LISW) by Ohio Counselor, Social Work, Marriage & Family Therapy Board
• Licensed Professional Clinical Counselor (LPCC) by Ohio Counselor, Social Work, Marriage & Family Therapy Board
• Licensed Independent Marriage and Family Therapist (IMFT) by Ohio Counselor, Social Work, Marriage & Family Therapy Board
