Job Summary:

Optimized Senior Living Group, Inc. is seeking a dedicated Licensed Practical Nurse (LPN) to join our Kettering team. This position is essential in providing high-quality care to our residents, ensuring their health and well-being. The LPN will work closely with the healthcare team to implement care plans, monitor residents' health, and provide compassionate support. This role requires working every other weekend, offering a consistent yet flexible schedule.

Key Responsibilities:

• Administer medications and treatments as prescribed by physicians, following all protocols and regulations.
• Monitor residents’ vital signs, health status, and respond to any changes in their condition.
• Assist with the development and implementation of individualized care plans in collaboration with the healthcare team.
• Provide direct nursing care, including wound care, catheter care, and other necessary medical interventions.
• Support residents with activities of daily living (ADLs) as needed, ensuring their comfort and dignity.
• Document all care provided, including observations, interventions, and resident responses in the medical record.
• Communicate effectively with residents, families, and other healthcare team members to ensure coordinated care.
• Participate in care plan meetings, contributing to the evaluation and adjustment of care strategies.
• Ensure compliance with all safety, health, and infection control policies and procedures.
• Provide leadership and guidance to caregivers and other support staff.
• Oversee completion of daily assignments/tasks. Assist on the floor as needed with resident care, meals, showers etc.
• Assist caregivers on the floor with daily tasks as needed. Assist with cooking, cleaning, resident care etc.

Qualifications:

• Valid LPN license in the state of Ohio.
• Previous experience in a long-term care, senior living, or similar healthcare environment.
• Strong clinical skills with the ability to manage multiple priorities in a fast-paced setting.
• Excellent communication and interpersonal skills, with a focus on resident-centered care.
• Ability to work every other weekend as part of the regular schedule.
• Compassionate and patient demeanor, committed to the well-being of residents.
• Physical ability to perform nursing duties, including lifting, transferring, and providing direct care.
