Job Title

Social Worker for Healthcare for Homeless Veteran Program

Job Description

The social worker is responsible for assessing needs and providing case management for Veterans in the Healthcare for Homeless Veteran Program (HCHV). The incumbent will work with the HCHV Program Coordinator to assess needs and assist housing insecure Veterans in identifying permanent housing placements. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD-VASH placements.

The incumbent performs site visits and assists Veterans in securing HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes. The incumbent verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation. The incumbent is responsible for ensuring that homeless Veterans are considered as primary participants for program acceptance. The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.

The incumbent completes assessments, case management and support in identifying permanent placement for housing insecure Veterans. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies.

The social worker complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-VASH Social Worker. The incumbent actively participates in the Healthcare for Homeless Program (HCHV) conference calls and attends training offered by the HUD-VASH. The incumbent is responsible for following local policy and SOP for the HUD-VASH Program.

The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. The incumbent takes responsibility for improving professional skills, participating in planning, and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.

The incumbent performs accountability functions by documenting all contacts with patients/families in the medical charts and completing statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies. The incumbent works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program Veterans.

The work is primarily community based. Consequently, the incumbent must operate a government vehicle to visit Veterans in the community and periodically transport Veterans in the government vehicle. Performs additional duties as assigned.

Work Schedule

Full-Time, Monday-Friday 8:00am- 4:30pm

Pay

Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade)

Paid Time Off

37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience

Parental Leave

After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child

Child Care Subsidy

After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
