The Registered Nurse serves as the case manager for assigned patients and as the teacher for patients’ caregiver(s). The RN case manager utilizes the nursing process and the six phases of the Alive CARE visit to provide safe, end‑of‑life care to the terminally ill patient/family unit. The primary responsibility is to coordinate and provide hospice care for home care patients through the development, initiation and updating of a plan of care in collaboration with patient and family.
RN case managers provide direct patient care, oversee the interdisciplinary team and teach patients and families regarding the disease process, symptom management and offer support during difficult times. Care delivery takes place in the patient’s home or any other facility where the patient may reside or receive care. The RN case manager will be accountable for meeting Alive Care Standards and participating in an on‑call rotation.
- Present a Professional Image:
Represent Alive positively, obey dress code, maintain car cleanliness, drive with etiquette, prepare for visits, and present calm and confident. - Provide Excellent Customer Service:
Conduct visits professionally and courteously, foster warm interactions with facility staff, patient and family, and use purposeful conversation, body language, and active listening. - Assess, Plan, Intervene and Educate:
Use critical thinking and the nursing process to assess patient and family, develop and implement an interdisciplinary hospice plan, initiate interventions, evaluate effectiveness, and educate staff and family/caregiver(s) on the plan, interventions, diagnosis, prognosis, medications, equipment, and how to contact Alive for assistance. - Document at the point of care:
Complete documentation verifying visit frequency, accurately reflecting the patient’s condition, update the plan as needed, and ensure staff have documentation of care. - Create a lasting impression:
Conclude visits with encouragement, provide the emergency phone number and supplies, practice active listening, and express gratitude. - Complete post‑visit follow‑up:
Send follow‑up messages to team members, synchronize iPad, and perform the spiritual release of the visit.
- Education:
Registered Nurse with a current nursing license; BSN preferred. - Experience:
Minimum one year of nursing practice or equivalent; experience in hospice, oncology, home health or long‑term care preferred. - Certificates, Licenses, Registrations:
Current Tennessee RN license or compact license; hospice certification desirable. Driver license and automobile insurance required if driving duties are involved.
