Job Overview

The Good Care Helpers is seeking a compassionate, dependable, and skilled Pediatric Licensed Practical Nurse (LPN) to provide full-time, one-on-one nursing care to a pediatric patient in the patient's home.

The LPN will implement the physician-ordered plan of care under the supervision of a Registered Nurse and help the patient remain safe and medically stable ponsibilities may include medication administration, enteral feeding, respiratory care, seizure monitoring, personal care, and other skilled nursing services based on the patient's individual needs.

This is a full-time position with regularly scheduled in-home shifts.

Primary Responsibilities
  • Provide one-on-one skilled nursing care in the pediatric patient's home.
  • Implement the physician-ordered plan of care under the direction and supervision of a Registered Nurse or physician.
  • Monitor and document the patient's condition, vital signs, pain level, respiratory status, nutritional status, intake and output, and response to treatment.
  • Administer prescribed medications, enteral feedings, treatments, and other nursing interventions within the LPN's authorized scope of practice.
  • Provide care for feeding tubes, tracheostomies, ventilators, oxygen, suction equipment, nebulizers, seizure disorders, and other complex medical needs when applicable.
  • Observe for changes in the patient's condition and promptly notify the supervising RN, physician, parent or guardian, and other appropriate personnel.
  • Follow the patient's emergency plan and initiate emergency procedures when required.
  • Assist with activities of daily living, mobility, positioning, hygiene, and comfort while encouraging the patient's highest possible level of independence.
  • Maintain infection-prevention and safety standards within the patient's home.
  • Verify medications, supplies, and medical equipment at the beginning of each shift and report shortages, malfunctions, or safety concerns.
  • Provide education and reinforcement to parents and caregivers regarding the established plan of care, medications, treatments, equipment, and emergency procedures.
  • Communicate professionally with the patient's family, supervising RN, physicians, therapists, case managers, school personnel, and other members of the care team.
  • Complete accurate, timely electronic clinical documentation for each shift.
  • Document medications, treatments, skilled interventions, changes in condition, caregiver communication, and the patient's response to care.
  • Participate in supervisory visits, case conferences, competency evaluations, and care-plan reviews as required.
  • Maintain patient confidentiality and comply with HIPAA, agency policies, professional nursing standards, and all applicable state and federal requirements.
  • Remain alert and attentive to the patient throughout the assigned shift.
  • Perform other patient-specific skilled nursing duties authorized by the plan of care and within the LPN's scope of practice.
Required Qualifications
  • Current, unrestricted Licensed Practical Nurse license in the state where services are provided.
  • Graduate of an approved practical nursing program.
  • Current CPR/BLS certification.
  • Pediatric nursing experience preferred.
  • Home-health, private-duty nursing, or medically complex patient-care experience preferred.
  • Ability to safely perform the clinical skills required by the assigned patient's plan of care.
  • Successful completion of all patient-specific competency evaluations before providing independent care.
  • Knowledge of pediatric vital signs, developmental stages, infection prevention, medication administration, and emergency response.
  • Ability to recognize and promptly report changes in a patient's condition.
  • Strong clinical documentation, communication, organization, and time-management skills.
  • Ability to work independently in a patient's home while maintaining appropriate communication with the supervising RN.
  • Reliable transportation, valid driver's license, and current automobile insurance.
  • Ability to successfully complete required background checks, health screenings, orientation, and training requirements.
Preferred Experience

Experience caring for pediatric patients with one or more of the following:

  • Gastrostomy or nasogastric tubes
  • Enteral feeding pumps
  • Tracheostomies
  • Ventilator dependence
  • Oxygen or respiratory treatments
  • Suctioning needs
  • Seizure disorders
  • Cerebral palsy
  • Developmental or intellectual disabilities
  • Neurological or genetic conditions
  • Other complex or chronic medical conditions
Schedule and Employment Status
  • Full-time position
  • Regularly scheduled in-home shifts
  • Schedule may include daytime, evening, overnight, weekend, or holiday shifts depending on the patient's authorized hours and care needs
  • Continued assignment is subject to patient authorization, census, staffing needs, and availability
  • Reliable and consistent attendance is an essential function of this position
Physical Requirements
  • Ability to stand, walk, sit, bend, reach, and perform nursing duties throughout the assigned shift.
