Responsibilities

• Provide direct nursing care to inpatient rehab patients, including assessments, medication administration, and care planning.
• Collaborate with physicians, therapists, and interdisciplinary team to develop and update individualized rehab care plans.
• Monitor patient progress, document outcomes, and adjust interventions to support optimal functional recovery.
• Educate patients and families on rehab goals, safety, discharge planning, and home care needs.
• Ensure compliance with safety, infection control, and quality standards within the rehab unit.
• Participate in shift handoffs, interdisciplinary rounds, and care conferences to coordinate care.
• Support admission, transfer, and discharge processes, including patient evaluation and documentation.
• Mentor and support nursing colleagues and ancillary staff to promote a collaborative team environment.
