Coverage Area:Jefferson, St. Clair, Blount, Cullman, and Etowah Counties
Mileage Reimbursement:Yes, reimbursed between patient visits
Position OverviewWe are seeking an experienced and compassionate Registered Nurse (RN) to provide high‑quality hospice care throughout the assigned coverage territory. The ideal candidate will have hospice nursing experience, strong assessment skills, and the ability to manage a daily patient caseload while delivering patient‑centered end‑of‑life care.
Key Responsibilities- Manage a caseload of approximately 5–6 patients per day.
- Conduct comprehensive nursing assessments in patients' homes and facility settings.
- Develop, implement, and update individualized care plans.
- Coordinate and communicate with physicians regarding patient status, treatment plans, and required orders.
- Provide skilled nursing care and symptom management for hospice patients.
- Educate patients, families, and caregivers regarding disease progression, medications, and comfort measures.
- Complete accurate and timely clinical documentation.
- Travel between patient homes and facilities within the assigned service area.
- Participate in rotating backup on‑call coverage as scheduled.
- Collaborate with the interdisciplinary hospice team to ensure quality patient care.
- Active Registered Nurse (RN) license in the State of Alabama.
- Current CPR certification.
- Previous Hospice Nursing experience required.
- Experience with Homecare Homebase (HCHB) or similar EMR systems.
- Strong documentation and organizational skills.
