Job SummaryThe Registered Nurse Care Team Leader (Case Manager) provides professional nursing services in accordance with physicians’ orders or plan of care to patients age 18 and older for a wide range of diagnoses. The RN, CTL is responsible for determining the nursing needs for an assigned group of patients/residents, initiating the plan, and evaluating the effectiveness of the plan of care.
The role supports the Village mission and values and completes required training hours.
- Effectively and timely provide comfort and well‑being of patients by assessing conditions, processing physicians’ orders, administering prescribed drugs, and providing treatments.
- Coordinate nursing care for patients and educate patients/residents and family about the care.
- Delegate appropriate aspects of nursing care to staff, evaluating, monitoring and assuring delivery of care.
- Accurately document patients/residents progress toward goals and other observations.
- Report patients/residents changes, needs and/or requirements to the appropriate staff.
- Perform other responsibilities as assigned by the supervisor to meet business needs.
- Maintain significant drive time in personal vehicle.
- Adhere to all safety regulations and requirements.
- Handle stress well and maintain appropriate workplace conduct.
Education: Associate’s degree in Professional Nursing (ADN) or higher.
Licenses /
Certifications:
- Registered Nurse in Missouri and Kansas.
- Current Basic Life Support (BLS) certification (or ability to obtain prior to direct patient contact).
- Valid driver’s license and proof of insurance.
- Basic level knowledge of Microsoft Windows and Microsoft Office (Word, Excel, Outlook).
- Ability to manage multiple tasks and responsibilities simultaneously.
- Work autonomously, particularly in Home Health and Village House settings.
- Read, analyze, and interpret general business periodicals, professional journals, technical procedures, and governmental regulations.
- Write reports, business correspondence, and procedure manuals.
- Present information effectively and respond to questions from managers, clients, customers, and the general public.
- Perform basic math calculations (addition, subtraction, multiplication, division, percentages, proportions).
- Define problems, collect data, establish facts, and draw valid conclusions.
- Work in inside and outside environmental conditions.
- Exposure to infectious diseases.
- Work with individuals who have mental capacity challenges.
- Drive between locations as required.
- May have on‑call responsibilities.
- May assist other departments during declared emergencies.

- Lift up to 50 lbs occasionally, 20 lbs frequently, or 10 lbs constantly.
- Require close visual acuity for close‑distance tasks.
- Require hearing at normal speaking levels.
- Perform frequent bending, kneeling, and crouching.
- Reach, push, pull, lift using upper extremities and back muscles.
- Use repetitive wrist, hand, and finger motions.
Receives daily supervision from the Clinical Manager.
