Location: Lee's Summit
Signify Health, part of CVS Health®, is seeking a full-time Nurse Practitioner to perform in-home health evaluations for members in their homes, using an iPad and clinical workflow.
Visits last about 45 minutes, include a medication and medical history review, a physical evaluation, and optional diagnostic tests if requested by the health plan. Candidates must hold an active, unrestricted license and board certification; multi-state licenses are preferred, with local travel required.
